Wellness Exams

(Medicare and Adult)

The Annual Wellness Visit (AWV) is a yearly appointment with your primary care provider (PCP) to create or update a personalized prevention plan. This plan may help prevent illness based on your current health and risk factors. Keep in mind that the AWV is not a head-to-toe physical.

Well Child Checks

Children need regular preventive care from their primary care provider, sometimes called wellness visits or well-child visits.

These checkups may include blood tests, height and weight measurements, and vaccines and will help track and protect your child’s health as he or she grows into an adult. These visits are generally covered at 100%.

Childhood Immunizations

We are a VFC participating provider.

Idaho Immunization Program (IIP) Administers the federal Vaccines for Children (VFC) program in Idaho. All vaccines recommended by the Advisory Committee on Immunization Practices (ACIP) are supplied at no cost, through a combination of federal and state dollars, for Idaho children 0 through 18 years of age.

Chronic Conditions

We focus on building relationships with our patients for better health outcomes. You and your provider will create an individualized health plan addressing your specific health care needs, from general wellness to the treatment and prevention of chronic illnesses.

Medication Management

Diabetes

Diabetes is a problem with your body that causes blood glucose (sugar) levels to rise higher than normal. This is also called hyperglycemia. There are different types of diabetes – type 1, type 2, and a condition called gestational diabetes which happens during pregnancy.

If you have diabetes, your body either doesn’t make enough or insulin, it can’t use the insulin it does make very well, or both.

If you have any concerns about diabetes such as a family history, any signs or symptoms or if you have been diagnosed with diabetes, come see us for testing and treatment. We offer in office A1c testing in a matter of minutes.

Every year, thousands of adults in America suffer serious health problems from diseases they could be vaccinated against like shingles, whooping cough, hepatitis A and B, flu, and pneumococcal disease. Smoking Cessation

The CDC reports that smoking leads to disease and disability and harms nearly every organ of the body.

  • More than 16 million Americans are living with a disease caused by smoking.
  • For every person who dies because of smoking, at least 30 people live with a serious smoking-related illness.
  • Smoking causes cancer, heart disease, stroke, lung diseases, diabetes, and chronic obstructive pulmonary disease (COPD), which includes emphysema and chronic bronchitis.

Long-term smoking cessation substantially reduces health risks. We offer resources and support that have been found to be effective in helping smokers to quit. Easy access to such smoking cessation treatment and support has shown to increase quit rates.
